Responsibilities:

End-of-Life Programme for Advanced COPD, Heart Failure and End-stage Renal Patients

You will report to the Palliative Care Consultant and Medical Officers.

* Provides comprehensive, coordinated, compassionate and holistic patient care.
* Develops, establishes and implements a plan of care in collaboration with patient and family.
* Supports the palliative care physicians in optimising pain and symptom control in patients.
* Provides education on patient’s condition, basic pain and symptom assessment and management, medication management, comfort measures, as well as, options of care, and available resources and services in the community.
* Facilitates effective communication between patient, family and the interdisciplinary team.
* Promotes continuity of care and services for patients.
* Arranges for medical equipment, supplies, or medications for patients.
* Facilitates and coordinates transfer of patient’s care to different levels or settings.
* Coordinates and participates in interdisciplinary team conferences.
* Ability to write reports of time spent, referrals, assessments, patient outcomes, evaluation on pain and symptoms and related activities.
* Participates and assists in research or studies involving end of life care.
* Performs other duties as assigned by the palliative care consultant and team.

Requirements:

* Registered Nurse with minimum 3 years of clinical experience in palliative care settings
* May be required to do after-office or weekend calls
* Ability to conversant in local dialects
* Knowledge on data management
* Ability to work as part of an interdisciplinary team, and have excellent interpersonal and communication skills
